What is a disability certificate?

The Disability Certificate is not just a document for a person with disability, but a proof of his/her disability and an important tool for availing the benefits / facilities / rights that they are entitled to, from the Federal as well as Provincial Government under various appropriate enabling legislations.

How to obtain Disability Certificate:

The procedures for obtaining a disability certificate are different for the Federal i.e. Islamabad and the provinces.

Obtaining Disability Certificate Federal (Islamabad)

  1. Get a form from CRPD (Council for the Rehabilitation of Disabled Persons)
  2. Fill the form and submit with the required documents.
  3. You will be called for “Medical Assessment bored” at National Institute of Rehabilitation Medicine (NIRM).
  4.   After 1 month you can receive your “Disability certificate” from NCRDP

How to obtain a disability certificate in the provinces (AJK, Balochistan, GB, KPK and Sindh).

Disability certificates are provided by the District Assessment Board in every district headquarters hospital.

Get a form from “District Social Welfare Office” or “District Headquarter Hospital”.

  1. Fill up the form, and submit along with 3 attested passport size photographs and relevant documents.
  2. They will inform you about the date of “Medical Assessment Board”.
  3. On due date, the Medical Assessment Board will physically examine persons with disabilities and decide if he/she is disabled or not.
  4. You will be provided with a “Disability certificate” after 1 month

How to get Special ID Card:

Special CNIC (Computerized National Identity Card with disability insignia) is a prerequisite for availing any benefit or entitlement offered by the government for persons with disabilities.

How to get Special ID Card:

Persons with disabilities can get Special CNIC from any of NADRA (National Database and Registration Authority) offices throughout Pakistan on the basis of disability certificate attested by Medical Superintendent or Chairman Medical Board of the government hospitals

Eligibility:

Every Person with a disability having “disability certificate”, a citizen of Pakistan and aged 18 or above is eligible for the Special CNIC.

Procedure:

Applicant who want to make his/her CNIC has to visit the NADRA Swift Registration Centers known as NSRCs. There are mainly two types of applicants:

  • One are those who are making their cards for the first time and are fresh applicants
  • The other one are those who had made their Special CNICs and they are applying for the second time due to any reason (Duplicate/modification/correction)

Required documents:

The following documents are required for the Special CNIC:

  1. Birth certificate
  2. Disability Certificate
  3. Educational certificates and transcripts
  4. National identity cards of your parents.
  5. You also need to bring either your parents’ Children Registration Certificate (CRC), or their Form B. Alternatively, if any one of your immediate family members already possesses a CNIC, then all you need is their CNIC number.

Assistive devices, such as wheelchair, white cane and hearing device, can be obtained from Bait-ul-Maal free of cost. Applicant requires a certificate from local Nazim confirming that the person with disability is of limited resources. The person must also be registered as a person with disability and must attach a disability certificate. The following is the contact detail:

A Pakistani national suffering from physical disability (excluding mentally retarded/handicapped and old age disability) may apply under the Disabled persons Scheme for import, once in 10 years, a new car not exceeding an engine capacity of 1350 cc for personal use without payment of custom duties provided the applicant, hereinafter referred as the “disabled person”
